What documents do I need to apply?

You’ll need three things to apply: proof of identity, proof of address, and proof of eligibility.

What to prepare

  • Identity: driver’s license, passport, birth certificate, citizenship certificate, or permanent resident card
  • Address: government-issued ID, utility bill, lease or mortgage document, or a benefit statement showing your current address
  • Eligibility: a program benefit letter, or proof of income at or below 135% of the Federal Poverty Guidelines

Tips for faster approval

Make sure documents are current, legible, and show your full name and address. Mismatched or expired documents are the most common cause of delays.

If your proof document will not upload

Use a clear image or PDF that shows the full document, your full name, and any required date or address information. If the file is too large, try taking a new photo, reducing the file size, or uploading from another browser or device.
